A Request for a New Product and/or Product Trial Approval Form should be completed and approved prior to the introduction of any new products or equipment for trial. Approval will be granted by the HSSBC Category Lead in cooperation with Standardization and/or the Biomedical team where appropriate.

Under no circumstances are vendors to provide unapproved product samples directly to or for patient use or leave materials, supplies or equipment on loan for trial, evaluation or any other purpose unless authorized by HSSBC on issuance of a term based purchase order for trial/evaluation only.

If loan/trial/evaluation equipment requires cleaning/disinfecting/sterilizing, the vendor must provide explicit instructions on cleaning methodology prior or upon delivery to reprocessing staff.
